			<content:encoded><![CDATA[<p>Next to my study I&#8217;m making some money once in a while with being gp-assistant and doing nursing tasks for people in their home situation (e.g. helping them eat, washing and dressing them, give medication etc.). While doing the last the other day, i found myself in quite an interesting situation: </p>
<blockquote><p><font color="#808080">The client was a ca 40 year old man, who&#8217;s more or less restricted to bed rest for a terminal illness. I had to apply a crème on his intertrigo/shaving lesions to cure them / prevent them of getting worse. Since i didn&#8217;t know this client, i asked him for some directions on where to find his lesions. Here is where it gets interesting. He wanted me to apply the substance on his bum-cheeks, his groins (very normal so far) and his prostate&#8230;&#8230;..?? Instantly my brain started to evaluate the situation. Did this guy ask me to do a, for this circumstance, very inappropriate act; or did he have no clue on where to find his own prostate? (In case you&#8217;re not familiar with this, the only way for me to reach his prostate in this situation was to insert my finger in his ass&#8230;) Taking everything into account I decided it was not fare to judge the guy for the first option if i didn&#8217;t rule out the second one first. So i had to make clear what exactly he meant with prostate. Luckily for the situation he meant his scrotum, and i managed to find this out without embarrassing him with his lack of knowledge of his own anatomy.</font>&nbsp;</p>

<p>Though i think this little anecdote is a quite funny example, i sometimes wonder about the lack of knowledge people have of their own bodies. I think that people would respect their body a lot more if they were roughly more aware of (the function of) their own organs. And if they have more respect for this beautiful system, maybe they will take care of it a little bit better&#8230;</p>
<p>There you are, that was my little reflection for today&#8217;s post.</p>

<p>As the icu doc describes it himself this blog gives its reader updates from the front lines of medical care. Reports of one doctor&#8217;s daily struggle in an Intensive Care Unit, as he fights the Angel of Death over the fate of the sickest patients in the hospital.
<p>It may be because so far intensivist (icu doctor) is very, very high on my list of what i want to do with my medical degree. It may be because it’s medical. But there is no doubt that his posts keep my attention. I like his style of writing, how he uses the angel of death (metaphor or not) to reflect the struggle of his “job” in an interesting and philosophical way.
<p>The angel will always win, not every battle, but in the end. As a icu dotor , day in day out, he is being confronted with the delicacy of live. No matter what, in the end there is nothing but the angel of death.

			<content:encoded><![CDATA[<p>Hmmm, i’m pretty slack on writing posts. Guess me having a blog has more to do with having a geek boyfriend than with being a good writer with hypes of inspiration. I keep on trying though; don’t want to give up yet. Maybe my internship will provide me with some good stories.
<p>After waiting 2.5 months, I finally started my internship. First 5 weeks we practice on other students and on trained patients. It seems kind of weird having study mates examine your body. Since year one the physical examinations are coming closer step by step, like the final exams in high school, dark and heavy. But, as we say in Dutch, “one can get used to anything”. And yes, even getting a breast examination of the guy you just had lunch with is not that weird at all.
<p>Today i did my first genital and rectal examination (just for the record, not on study mates but with trained coaches). I’m very happy to practice it before i have to examine a real patient in the clinic. So to everyone doing this, thanks a lot for teaching us!!!!! On behalf of me, and my future patients maybe even more! Previous to this lesson I was a bit nerves about the genital and rectal examination. What to expect? Isn’t it weird to do, it seems to be so intimate? Well, i can assure any one, for the examiner it is just as technical as examining any other part of your body, and just as normal.
<p>Ok, enough of all this physical stuff now. I’m off to study some more before i go to bed. Early start tomorrow as I have to be in the hospital 7:15 to practice vena punctures (drawing blood).

<p>Anyway, we had a wonderful time, Daniels parents looked after us (thanks so much!!), and did a very good sales job on Adelaide. I met hypes of Daniels friends and family and we have seen quite a few things as well (Adelaide, Victor Harbour, Granite Island, wineries, Barossa Valley, Clair Valley, Flinders ranges, Kangaroo Island, great ocean road, Melbourne, Sidney). Thanks every one for the good entertaining, come to Amsterdam! (Not all at the same time though, Amsterdam houses are noting like the ones in Australia ;-))</p>

<p>M&#8217;n eerste post vanuit Australie, 10:45 uur lokale tijd, 03:15 uur voor iedereen in Nederland. Met ons gaat het super goed, we doen alleen maar leuke dingen.
<p>De reis is allemaal goed gegaan, deur tot deur heeft het ca. 40 uur (!!) geduurd. Eerst van Amsterdam naar London waar we een aantal uur moesten wachten op de volgende vlucht. Het tweede deel ging van London naar Bankok. In bangkok werd er een security check uitgevoerd (niet schrikken mam, normale procedure) en er werd geladen en gelost. Daarna naar Sydney, waar we op het vliegveld zelf, al wachtende in de rij, zoveel vertraging opliepen dat we de aansluiting naar Adelaide misten. Grrrr, net wat je nodig hebt na 35 uur onderweg te zijn geweest. Afin we zijn goed en wel in Adelaide aangekomen. En ik schaamde me tegen die tijd allang niet meer voor de vreselijke broek die ik aan had, daar zat ie veel te lekker voor. Had niet graag al die tijd in een spijkerbroek opgevouwen in dat stoeltje willen zitten. En het is best wel een grappige gewaarwording, we gingen op een zondag weg en kwamen op dinsdag aan, maar voor je gevoel is het nog steeds de zelfde dag. Een heel raar besef van tijd.
<p>Daniels ouders en James (broertje) kwamen ons van het vliegveld ophalen. Iedereen was echt super blij om elkaar weer te zien! De rit naar huis (maar een half uurtje ofzo) was dan ook heel gezellig.
<p>Mijn god wat een ruimte is daar zeg! Dorpen in Nederland hebben nog minder ruimte dan Adelaide, welke de 5e grootste stad van Australië is. En Daniel heeft niet overdreven, hun zwembad heeft inderdaad ongeveer de oppervlakte van onze achtertuin (niet in A&#8217;dam maar bij papa en mama thuis). Ik zal binnenkort wat foto&#8217;s van de tuin en het huis posten.
<p>Daniels ouders hebben ons echt heel liefs ontvangen. Op ons bed in Daniels oude kamer lagen nieuwe badjassen en handdoeken klaar, Australische badproducten en boekjes van dingen die we gaan bezoeken. De eerste middag hebben we wat rondgekeken in en om het huis en &#8217;s avonds was het hele gezin met aanhang compleet voor een diner. Zo leuk om eindelijk Daniels broertje en zus met aanhang te zien. En super veel gelachen. &#8217;s Avonds moesten we al vroeg afhaken aangezien we beiden nogal gebroken waren. Weinig geslapen en veel indrukken.
<p>De volgende ochtend kregen we ontbijt op bed!! jammie jammie en gingen we naar Victor Harbor om een beetje bij te komen. Ongeveer 70 Km van Adelaide vandaan. Het plaatsje ligt aan de kust (adelaide trouwens ook). Onderweg hebben we een aantal wijn proevereitjes gedaan, en wat mooie dingen bekeken. We zijn op Granite Island geweest en sliepen op marshall island.
<p>Vrijdag en zaterdag hebben we wat van Adelaide bekeken en een bikini gekocht zodat we eindelijk onze eerste duik in het zwembad konden maken. Zaterdag avond hebben Daniel en ik zijn neefje van acht die een nachtje kwam logeren meegenomen voor een avondje uit. Daniel en Jack zijn echt reuze gek op elkaar.
<p>Zondag organiseerde Debbie en Wayne een barbecue ter ere van Daniel en mij voor 50 mensen. Het was heel gezellig en leuk om eindelijk gezichten bij alle namen en verhalen te hebben.
<p>Voor maandag en dinsdag stond flinders ranges op het programma, helaas kon Debbie niet mee omdat ze weer aan het werk moest. Flinders ranges ligt noordelijk ten opzichte van Adelaide en is een bergachtig gebied met een aantal natuur reservaten en grote stukken landbouw grond. Het is allemaal zo immens groot, we hebben uren rondgereden zonder mensen tegen te komen. We hebben kangoeroes, emu&#8217;s, een arend en een hoop kleine dieren gespot. We in een dorpje beland wat eens 5000 inwoners had en waar er nu nog maar 150 van over waren. Echt een spook stadje. Voor de rest hebben we een aantal andere interessante dingen bekeken.
<p>De terug weg zijn we door de clare valley en de barossa valley gegaan, de laatse is het grootste wijn gebied in Australië.

<p>Daniel met Coky, de vogel. Alle twee op de zelfde tijd aan het gillen. De vogel herhaald bepaalde woorden en als je hem alleen laat of een bepaald geluid maakt begint hij te schreeuwen. Erg leuk beestje.</p>

<p>Voor de boeren eindelijk regen in de extreme droogte van de laatste tijd. Voor ons gaaf om te zien en voor de inwoners, erg vervelend. Grote onweers stormen overspoelen in korte tijd delen van het noorden van zuid austalie. In een dorpje viel 50 mm in 30 minuten.</p>
